Wellness Nurse

About This Role

We are looking for a Wellness Nurse to join our team. In this role, you will focus on promoting health and wellness among our residents. You will conduct health assessments, manage chronic diseases, and provide education.

What You'll Do

• Perform comprehensive health assessments for residents.
• Develop and implement chronic disease management plans.
• Educate residents and staff on health and wellness topics.
• Coordinate care among different healthcare providers.
• Design and facilitate wellness programs.
• Offer nutritional counseling to support healthy living.
• Manage medication for patients as needed.
• Delegation for care staff med pass

What We're Looking For

• Mid-level experience in nursing, preferably with 3-4 years in a similar role.
• Valid RN or LPN certification.
• Strong skills in health assessments and patient education.
• Experience with chronic disease management and care coordination.

• Base salary ranging from $69,000 to $95,000 per year.
