Job description
MD at Home has been providing care for more than 15 years, our home-based primary care practice has been dedicated to serving low-income, chronically ill older patients across Northern Illinois, Northwest Indiana, and South Florida. Our practice centers on enhancing patient outcomes by delivering comprehensive clinical care paired with strong operational support.
Patient Population and Schedule
Primarily geriatric and medically complex individuals with chronic conditions including diabetes, COPD, CHF, hypertension, and dementia. Residing in private homes, independent living facilities (ILF), assisted living facilities (ALF), or senior living facilities (SLF)
- We typically handle a daily caseload of 8–10 patients, with intervals of 2–10 minutes between appointments.
- Routine follow-up visits generally take about 15 minutes, depending on the risk level of the patient.
- Initial appointments for new patients are allotted roughly 45 minutes, or longer as required.
- Within 45 minutes drive from your home
Credentials:
Active NP/PA/MD license and certificate in state of FL
Driver’s license
DEA license (obtain if needed) / drug enforcement administration
Strong clinical skills and communication
Experience in primary care, geriatric, family medicine. And home-based care preferred
