DMD Pathways is seeking an independent registered nurse consultant to provide family navigation and trial-specific preliminary prescreening services to families affected by Duchenne muscular dystrophy.
DMD Pathways leadership will review all incoming inquiries and determine which families should be referred to the consultant. The consultant will not monitor general inbound inquiries, manage a public-facing inbox, maintain scheduled coverage periods, or provide continuous on-call availability. Instead, DMD Pathways will offer individual assignments to the consultant as family needs arise. Assignments may involve general family navigation, treatment-navigation support, or preliminary prescreening for a specific clinical trial.
The consultant may accept or decline individual assignments based on availability. Each family interaction will be scheduled according to the family’s needs and the consultant’s availability. No minimum number of cases or hours is guaranteed.
Primary Areas of Responsibility
Family Navigation
For assigned family-navigation cases, the consultant may:
- Review intake information and the reason for referral.
- Contact the family to confirm or arrange a mutually agreeable appointment time.
- Conduct scheduled navigation conversations by telephone or video.
- Listen to the family’s questions, priorities, concerns, and practical barriers.
- Provide general education and explain relevant information in clear, family-friendly language.
- Connect families with DMD Pathways resources, healthcare specialties, support organizations, financial resources, educational information, treatment-navigation services, and other appropriate services.
- Help families identify questions to discuss with their treating healthcare professionals.
- Support families in understanding the next steps within a healthcare, research, or support process.
- Provide warm handoffs to appropriate organizations, providers, research teams, or DMD Pathways services.
- Document resources provided, referrals made, follow-up needs, and outcomes.
- Complete agreed follow-up with the family.
- Escalate clinical, safeguarding, privacy, safety, or out-of-scope concerns according to DMD Pathways procedures.
