Responsiblities:
Title: RN Psych/Behavioural Health
Shift: Day Day Shift, 4 x 12s. no set days of the week. 0630 - 1900 & Night Shift, 4 x 12s. no set days of the week. 1830- 0700
Deliver comprehensive, patient-centered nursing care to assigned patients in accordance with the nursing process, evidence-based practice, and established standards of care.
Perform comprehensive patient assessments, collect and analyze clinical data, identify nursing diagnoses, and develop individualized plans of care.
Implement, coordinate, and evaluate nursing interventions to achieve optimal patient outcomes while ensuring patient safety and quality care.
Monitor patient conditions, recognize changes in clinical status, and initiate appropriate nursing interventions while communicating significant findings to the healthcare team.
Administer medications, IV therapies, blood products, and treatments safely and accurately in accordance with physician orders and hospital policies.
Collaborate with physicians, advanced practice providers, therapists, case managers, pharmacists, and interdisciplinary healthcare teams to coordinate comprehensive patient care.
Provide patient and family education regarding diagnoses, medications, treatment plans, procedures, discharge instructions, disease prevention, and health promotion.
Participate in discharge planning to ensure continuity of care, safe patient transitions, and appropriate follow-up services.
Maintain accurate, timely, and regulatory-compliant documentation of patient assessments, interventions, medications, care plans, and patient responses in the electronic medical record (EMR).
Prioritize patient care based on acuity while utilizing critical thinking, sound clinical judgment, and evidence-based decision-making in a fast-paced acute care environment.
Delegate nursing tasks appropriately to licensed and unlicensed personnel while supervising care delivery and ensuring compliance with nursing standards.
Maintain effective communication with patients, families, physicians, and multidisciplinary healthcare teams to promote positive patient outcomes and continuity of care.
