**About the company**

A Series A, seed-backed healthcare AI startup building agentic AI for utilization management, AI agents that read, reason over, and help adjudicate complex clinical decisions in real time. The product is already live and processing a high volume of real prior-authorization requests monthly, with measurable improvements in provider satisfaction. The founding team combines deep machine learning research and technical backgrounds, and the engineering team is small and talent-dense.

**The role**

You'll turn genuinely messy, high-stakes clinical workflows into working AI systems, building RAG pipelines, multi-agent systems, and structured extraction from unstructured medical records from day one. Because the output of these systems is audited by clinicians and regulators, "mostly right" is treated as a failed state here — this is a role for people who dig until they find the actual fix, not just a plausible one. You'll also become customers' most trusted technical partner, working directly with founders to help shape the product and technical direction as the company scales.

**What you'll do**

- Scope and architect AI solutions for ambiguous, high-stakes clinical problems, from the first conversation through a production-ready spec

- Build and deploy RAG pipelines, multi-agent workflows, and AI systems that process clinical medical records

- Ensure interoperability with healthcare data standards (EHR, FHIR, HL7)

- Own monitoring, performance tuning, and post-deployment improvement with genuine attention to detail

- Identify new opportunities for AI adoption within existing client accounts

- Partner with product and clinical safety teams to close feedback loops

- Navigate clinical, operational, and technical stakeholders with clarity and real credibility

**What you must bring**

- A strong GitHub profile with real, demonstrated work

- Proficiency in Python

- Infrastructure-as-code experience, such as Terraform or a similar tool

- 1+ years of relevant experience

- The instinct to kill bad ideas fast and refuse to accept requirements you don't fully understand

- A genuine need to understand how things work all the way down, not just at the surface

- Comfort working in-person, five days a week, in San Francisco

**Nice to have**

- Active open source contributions

- A GitHub history showing contributions prior to 2022

- Prior founding or startup experience

**Compensation & logistics**

- $120K–$190K base salary plus 0.1%–0.5% equity

- Stock options, health benefits (including dental and vision), and unlimited PTO

- Friday lunches in the San Francisco office

- Latest Mac laptop, substantial GPU access, and coding/chat-based AI assistants provided

- On-site five days a week in San Francisco

- 4 openings

- Open to visa sponsorship (H-1B, O-1 transfer)

**This isn't the right fit if**

- You're not currently employed

- You currently run your own consulting business rather than working within an established company or team

- Your work history shows short tenures or frequent job-hopping
