Join our dynamic healthcare team as a Physical Therapist, where you will play a vital role in enhancing the independence and quality of life for our geriatric patients receiving care in their homes. This is a flexible position that operates out of Monmouth County and Middlesex County in NJ.

We work closely with physicians, specialists, and care teams to deliver coordinated, patient-centered treatment without the burnout of high-volume clinics.

Responsibilities

• Provide one-on-one in-home physical therapy

• Evaluate patients and develop individualized plans of care

• Communicate and collaborate with referring physicians, specialists, and care teams

• Build strong relationships with patients and families to support optimal outcomes

• Coordinate care and maintain continuity between patients and providers

• Complete documentation accurately and in a timely manner

• Represent Elite Mobility Home Therapy with professionalism and compassion

Requirements

• Licensed Physical Therapist in good standing

• Strong communication and interpersonal skills

• Comfortable working independently in a home-based setting

• Patient-focused and relationship-driven

• Valid driver’s license and access to a reliable vehicle

• Home health experience is a plus, but not required
