Counselor/Case Manager

About This Role

We are looking for a Counselor / Case Manager to join our team. In this role, you will provide support to individuals in the re-entry population. Your work will be vital in helping clients navigate their way to independent living. 

What You'll Do

• Conduct counseling sessions with clients to address their goals and how they will reintegrate into society. 

• Maintain accurate documentation of client progress and treatment plans.

• Coordinate case management services to ensure clients receive comprehensive support.

• Engage with clients in both individual and group settings to promote recovery.

What We're Looking For

• High school diploma or equivalent.

• Certification from CCAPP/CADTP / ACCBC (preferably Certified but will accept Registered Counselors who are completing their hours) and CPR/First Aid.

• Strong counseling skills with the ability to connect with clients.

• Experience in documentation and case management.

• Proficiency in English and Spanish is a plus.
