Compensation: $105,000 – $142,000 (Total Compensation Range) + Up to $32,000 Annual Bonus

Role Overview
This is a high-priority leadership position for a qualified Registered Nurse (RN) focused on coordinating, leading, and supporting patient care services within a structured home health setting. Operating in a hybrid capacity out of San Antonio, TX, you will take complete ownership of caseload planning, referral reviews, clinical order approvals, and clinical staff alignment while strictly adhering to compliance and regulatory standards.
Key Responsibilities
Care Planning & Coordination: Coordinate, monitor, and oversee comprehensive care planning for all patients under your assigned caseload.
Referral & Order Review: Review all incoming new patient referrals for clinical appropriateness, and approve/co-sign clinical orders (including LVN and follow-up orders).
OASIS & Documentation Guidance: Provide strategic guidance on OASIS documentation and assist field RNs with structured utilization reviews.
Interdisciplinary Communication: Facilitate case conferences, maintain communication with physicians, schedulers, therapists, and field staff to ensure continuity of care.
Compliance & QAPI Coordination: Ensure timely submission of assessments, align documentation with billing guidelines, and coordinate corrections directly with the QAPI team.
Supervisory Support: Support the Director of Nursing by supervising field staff, assisting with scheduling, and participating in call rotation/triage responsibilities.
Required Qualifications
Core Experience Level: Minimum of one (1) year of recent clinical experience as a Registered Nurse (RN).
Home Health Expertise: At least one (1) year of dedicated Home Health experience is strictly required.
Operational & Regulatory Fluency: Thorough understanding of Home Health operations, Medicare Conditions of Participation (CoPs), and operational management of OASIS documentation.
Software Proficiency: Hands-on proficiency with Homecare Homebase (HCHB) software systems.
Screening Documents: Must possess a valid Driver's License for Screening and Selection Services (SASS) and complete a Candidate Attestation.
Preferred Qualifications
Previous leadership or supervisory experience within a home health or clinical care environment is strongly preferred.
Proven experience with utilization review, quality assurance, and formal performance improvement initiatives.
Working Conditions
Primarily office-based with occasional field responsibilities and local travel/patient home visits as needed.
Must be able to manage multiple concurrent tasks in a fast-paced environment.
Ability to lift up to 50 lbs and support patients physically when required.
